2018-2019 NJHS Community Service Record Sheet

 

NJHS members are to use this form to record and document community service hours for both outside of school activities and NJHS functions and events.  The hours must be validated by the supervisor of the event where you completed these hours.  In the event that this is impossible to achieve, please have the hours validated by a parent.

 

Please make certain to complete all areas of the record sheet, including your signature, total hours, and date.  If anything is incomplete, the record sheet will be returned to provide an opportunity for completion. 

 

When the form is completed, please return it to NJHS Vice President Kiley Williams.

 

Community Service Record Sheets are collected at each monthly NJHS Member Meeting.  

 

Important Note:  Please do not record the NJHS Member Meetings on your Community Service Record Sheets.  Attendance will be taken at the NJHS Member Meetings and that will be automatically added into your total service hours.
